Began Ciprofloxine 500mgm. prior to Biopsy.

10-Oct-97 Diag: TRUS Biopsy & Total body bone scan.
Results: 6 segment TRUS Biopsy Oct 10th, Prostatic Carcinoma in all segments, Adenocarcinoma. Gleason Grade 4+4, Gleason Score 8/10.
Verbal results of bone scan "Multiple Metasteses."

17-Oct-97 Rx
Started Morphine 5 gr prn. Requested copies of the lab work and ultrasound .

20-Oct-97 Event:
NoBM for several days in spite of Colase & Sennakot. Unable to void. Cannot keep fluids down. Rushed to hospital emergency; a foley catheter was inserted and a High soap-suds enema did the rest.

Written Report of whole body scan of Oct. 10/97 received.
The study shows multiple foci of increased activity scattered throughout the skeleton. There are lesions noted in the skull base,and the spine and multiple ribs, in the pelvis, both femora and in both scapula consistent with multiple skeletal metasteses.

Awoke to find catheter bag still dry & empty. To outpatients for removal of foley catheter. Still no urinary output. Still no BM. More bloodwork to ensure kidney function, another Ultrasound - all normal. Admission to hospital, I.V. began. Start Stemitil (ordered prn for nausea.)

26-Oct-97 Procedure:
TURP - URO states, " Prostate not overly enlarged, but was completely blocking passage"

27-Oct-97 Rx:
Start Casodex 50mg, - once daily.

31-Oct-97
Discharged from hospital on Casodex, Stemitil & MS Contin.30mg Proposed Lupron and Strontium-89 to begin November 3rd
On the advice of Dr S. Strum, (PCRI) and Dr Ian Tannock, Chief of Medicine at Princess Margaret Hospital we have withdrawn from the "Study"
